Hospice Physicians
As primary care providers, their work concentrates on reducing your pains and sustaining your comfort by developing a care plan that is also effective in meeting your personal goals.
Hospice Social Workers
They provide you and your family members counseling to cope with your condition and the feelings surrounding death. They can also connect you to resources if you need additional support and assistance.
Visiting Nurses
To ensure consistency in care, a nurse or team of nurses will check on you from time to time, administer your medication, and monitor the changes in your condition. They are responsible for the coordination of the hospice care team as well.
Home Health Aides
They are responsible for providing the non-medical routine care you need such as assistance in personal care (bathing, personal hygiene, and grooming) and eating. They work under the supervision of nurses and doctors.
Hospice Chaplains
They are responsible for meeting your spiritual needs. They provide counseling to cultivate your spiritual well-being by providing you with answers and consolation for your thoughts and feelings as you near the end of your life.
Therapy Consults
To further your comfort, our therapists will provide you with personalized treatments to either reduce your pains, ease your anxiety and stress about your condition, or improve your mobility or communication.
Hospice Volunteers
We have trained volunteers as well who can support you in a variety of non-medical areas to sustain your overall wellness. Some of them provide transportation assistance, companionship, or as respite caregivers.
